Steps to making successful medical claims

Claims Services are provided by TIC.

1. Call

To get authorization for medical procedures, including certain diagnostic tests (such as a CAT scan or MRI) call TIC at 1-800-995-1662.

You must call TIC immediately if you are admitted to a hospital.

2. Claim Form

You must bring a claim form with you if you seek medical attention at a doctor's office or walk-in clinic. Section E on the claim form should be completed by your attending physician.

3. Submit Claim

To process a claim TIC will need your completed claim form, along with all related bills/receipts and treatment statements.

Tip! Submit all claims as quickly as possible, and before you mail original documents, make copies for your own records.

Send claim forms, and all pertinent documents to the address indicated in the claim form.
