To get authorization for medical procedures, including certain diagnostic tests (such as a CAT scan or MRI) call Norfolk Mobility Benefits at 1-800-672-6089 (ext. 2909). When the operator answers ask for the claims department. If you reach a recording you will be asked to enter the extension you wish to reach. In this case enter 2909 to be transferred to the claims department. If you are hospitalized you must call TIC immediately. |
2. Claim Form |

Bring a claim form with you if you seek medical attention at a doctor's office or walk-in clinic. Ask your doctor to complete Section 3 on the claim form, or to tell you how to complete it. |
3. Mail Original Documents |

You must mail all original receipts for payments you have made. These could include the payment to a doctor/clinic, payment for prescription medication at a pharmacy, payment for a lab test or x-ray, payment for treatment at the out-patient emergency ward of a hospital. Before you mail original documents, make copies for your own records. Mail claim forms, and all pertinent documents to: Norfolk Mobility Benefits |

For a complete understanding of the procedures and conditions that apply to medical claims, read your policy wording, especially the section called “Procedures Applicable to Medical Claims.” Because the policy wording is a legal document some of the language may be difficult to understand.
