Enrolment

To enrol for the Global Campus Health Plan:

If applying for the Premier Plan you may apply online (with a credit card payment) using the Secure Online Form.

Otherwise, send your application form Premier Plan Application or Basic Plan Application with your payment to David Cummings Insurance Services (DCIS).

Mail applications (with a cheque, money order, or credit card payment) to:

Once your application has been received and approved by DCIS, we will send your policy kit (policy wording, ID card and claim form) to the Canadian mailing address indicated on your application form, or to the email address indicated on your application form.

Need to extend coverage?

If you need to purchase an extension of coverage please complete a new application form and clearly note that you have a policy which you wish to extend. For assistance please contact DCIS.



Family Members

If you have a spouse/partner and/or dependent children who will reside with, or visit you during your study term, you may purchase GCHP coverage for them.
